From the hospital to home

The transitions to hospice care at home can be stressful especially as families face the reality of a loved one’s condition or life-limiting diagnosis. The job of the hospice team is to make sure the patient and their family are getting the services, supplies and information they need to be comfortable, calm and confident in the convenience of their own home, wherever home may be.

Transitioning to hospice care may also include the use of special medical equipment—a wheelchair, hospital bed, oxygen—the hospice staff ensures it is delivered to the patient’s home and the family is fully instructed on its use.

Medications that are related to the Hospice Diagnosis are covered under Hospice. The INS nurse organizes medications, makes sure refills are ordered and received, and ensures proper disposal of medications that are no longer needed. With the INS hospice family supporting the transition to hospice, you and your loved one never have to go it alone.

You and your loved ones deserve the best hospice care can provide. Please let us know if there is any way we can offer our assistance by sending us a message or calling us at 678-806-5900.
